| In order for us to provide the public with credit card billing services
from a remote area, we contract with the VitalChek Network and they will charge you a shipping
and handling fee of $9.00 for expedited services. Once we authorize your request through
the credit card billing system, it cannot be canceled or changed. Therefore, be sure the
information you send is accurate.
Vital Statistics fees for Birth, Marriage and Divorce records are $12.00
for one certified copy and $7.00 for each additional copy of the same record issued at the
same time or search thereof. Death records are $13.00 for one certified copy and $8.00 for
each additional copy of the same record issued at the same time or search thereof. If you
do not know the exact date of the event, we will search 5 years for the record. You must
indicate the years to be searched. EXAMPLE: If you think the event happened sometime in
June of 1946, we will search the years 1944 to 1948. If you are not sure of the date at
all, specify the time frame you wish searched, i.e. 1950 - 1955. If you require more than
5 years to be searched, state the years and the order to search. EXAMPLE: 1950 – 1960,
start the search at 1960 and go back to 1950. If we do not locate the record, the $12.00
fee for Birth, Marriage, Divorce and $13.00 fee for Death will be retained for the search.
For personal family genealogy research, only five searches within
the same family group will be processed for the $9.00 VitalChek fee. (More than three listings
within the same request will be billed an additional $9.00 fee) EXAMPLE: If you list 6 different
family members, the VitalChek fee will be $18.00. A $9.00 VitalChek service fee for family
history or heir requests from professional companies who do family research for their customers
will also be charged for every five listings (or part thereof) within the same family group.
We are not authorized to issue copies C.O.D., send overnight carrier services
delivery C.O.D., or bill third party accounts. VitalChek gets a reduced rate for Federal
Express. Federal Express will deliver from our office only to an address where
someone is available to sign for the delivery.
To receive a certified copy(s) of a vital record, you should complete
the information page and FAX it to our office any time or day of the week. (785) 357-4332.
Faxed requests should be processed and mailed within three working
days of receipt. It is not necessary for you to call or send e-mail to verify receipt
of request, as most fax machines give you conformation that the fax was sent (inquiry calls
delay our processing). However, if you have not received your record within a reasonable
amount of time, call our office at (785) 368-6379. Remember, do not count weekends or authorized
holidays in the processing time, but do allow for the mail delivery time.
DO NOT SEND A REQUEST TO OUR OFFICE IF YOU WERE BORN IN ANOTHER STATE.
WE ONLY RECORD EVENTS THAT HAVE OCCURRED IN THE STATE OF KANSAS.
Only one attempt will be made to call the requestor if the credit card
is declined or the information is incomplete or illegible. (This does not apply to a busy
signal).
If you are inquiring from a foreign county, you must provide the name
and daytime telephone number for a contact person in the United States.
Your copy(s) will not be returned to you via FAX machine. We are
only authorized to issue State certified records, which are issued on a multi-colored safety
paper with a raised embossed state seal.

PUBLIC INSPECTION. VITAL RECORDS ARE AVAILABLE TO ONLY THE PERSON NAMED ON THE RECORD, PARENTS,
OTHER DIRECT RELATIVE (BROTHER, SISTER, AUNT, UNCLE OR GRANDPARENT), OR AN AUTHORIZED REPRESENTATIVE.
AUTHORIZED REPRESENTATIVES MUST SUBMIT PROOF OF INTEREST.
