You need to contact our office at (785) 296-1434 or smorriss@kdhe.state.ks.us to have us mail you a delayed birth packet that includes the delayed birth certificate form that needs to be completed and signed before a notary. In that packet, you will also receive instruction’s that explains how to file a delayed birth certificate and a list of suggested documentation.
