Filing a Report of Adoption on Child Born in Kansas

When an adoption has been completed on a child who was born in Kansas, a Report of Adoption or Certificate of Adoption form must be submitted to our office. The Report of Adoption form must be signed and certified by the clerk of the court or by the judge granting the adoption. It is not necessary to send the Decree of Adoption with the completed Report of Adoption or Certificate of Adoption form.

Once our office has received the completed form, we create a supplemental birth certificate which is sent to the attorney so that the adoptive parent(s) can look over the certificate and sign it. The certificate must be returned to our office within 30 days to be placed on file permanently. If the certificate is not returned within the 30 days, the certificate will be placed on file without the adoptive parent(s) signature. The filing fee is $12.00.

Once the certificate has been placed on file, certified copies or cards may be obtained for $12.00 for the first copy or card and $7.00 for each additional copy or card of the same record ordered at the same time.
