Heart to Heart Home Plus Fined $2,500

The Kansas Department of Health and Environment (KDHE) has issued an order assessing a $2,500 civil penalty against Heart to Heart Home Plus, 1004 S. Walnut, Ottawa, for violations of state adult care home regulations.

KDHE staff inspected the adult carefacility on October 31, November 1 and November 6-8 and issued a correction order on November 20 citing the facility for failure to meet standards for staff treatment of residents. KDHE staff conducted a follow-up inspection on December 3-5 that determined the deficiencies had not yet been corrected.

The department assesses a fine when a facility fails to correct the deficiencies by the second visit. Department surveyors continue to check the facility to ensure the deficiencies are corrected.
The order is subject to appeal to the Secretary of Health and Environment.
