2011-2012 SCHOOL & CHILD CARE IMMUNIZATION INFORMATION
4 DAY GRACE PERIOD ISSUE REGARDING BACK TO SCHOOL IMMUNIZATIONS-
When reviewing immunization records, note that vaccine doses administered up to 4 days before the minimum interval or age can be counted as valid. If doses are administered more than 5 days to early the person needs to be recalled for a valid dose. The repeat dose should be administered after the invalid dose by an interval at least equal to the recommended interval and/or age. The Kansas Certificate of Immunization utilized by Kansas schools to document students immunization records states that KDHE endorses the 4 day grace period as recommended by CDC. Please, do not refer persons that fall in this exception to the rule for a repeat dose. Immunization clinics should not be in the practice of routinely administering vaccines using the 4 day grace period. Vaccines should be administered at intervals that are recommended by Advisory Committee of Immunization Practices to ensure the best protection from disease. If a person falls behind on their schedule or if it is unlikely the child will return, the minimum interval may be used.
The 4 day grace period applies to all vaccines except Rabies.

POLIO VACCINE- IMPORTANT INFO FOR SCHOOL COMPLIANCE
The 2011 ACIP Immunization Recommended Schedule (http://www.cdc.gov/vaccines/recs/schedules/downloads/child/2011/11_0-6yrs-schedule-pr.pdf) reflects the 2009 updated recommendations of the ACIP regarding routine poliovirus vaccination (http://www.cdc.gov/mmwr/preview/mmwrhtml/mm5830a3.htm?s_cid=mm5830a3_e). The final dose in the IPV series should be administered at age 4 years or older and the minimum interval from dose 3 to dose 4 is extended from 4 weeks to 6 months.
Communication received from CDC stated that there was no recommendation to make this retroactive before August 7, 2009 when the recommendation was published. The 4 yr minimum age and 6 month minimum interval applies to current vaccination activity for completion of series for preschool and kindergarten children. The 6 month interval also applies to older children and adolescents who need a last dose for series completion. Older children and adolescents vaccinated according to the previous recommendations do not have to be revaccinated if the 4th dose met the previous recommendation of 4 wks minimum interval regardless of the age it was administered. Older children and adolescents with a history of 3 doses, the last of which was administered after age four and with a 4 wk interval from the prior dose can also be considered as compliant. Providers should not recall those individuals over 7 years of age with an invalid polio vaccine dose showing in the system at this time. Students currently enrolled in school should be considered compliant.
