HealthConnect Kansas
HealthConnect Kansas (HCK) is a statewide primary care case management program established in 1994 to provide Medicaid beneficiaries with access to quality medical care in an efficient and economical manner. The Division of Health Care Finance (DHCF) contracts directly with Primary Care Case Managers (PCCMs) who receive a per member per month (PMPM) fee to provide some components of a medical home. They also provide care coordination for specialty care referrals. Medical services obtained by HCK members are reimbursed on a fee-for-service (FFS) basis.

The Consumer Assessment of Healthcare Providers and Systems (CAHPS) program develops and supports the use of a comprehensive and evolving family of standardized surveys that ask consumers and patients to report on and evaluate their experiences with health care.
