HealthQuest Rewards Program
October 1, 2011 – July, 31, 2012
From October 1, 2011, through July 31, 2012, employees enrolled in the medical portion of the State Employee Health Plan (SEHP) have an opportunity to earn credits by participating in wellness activities offered through HealthQuest. Those who complete a health assessment worth 10 credits and earn an additional 10 credits by July 31, 2012, will earn an incentive. The incentive is a discount of $480 on their health plan premiums for Plan Year 2013.
The HealthQuest Rewards Program is optional and employees are not required to participate in order to be covered under the SEHP. There is no additional cost added to an employee’s premium for not participating, but they will be paying the base rate (undiscounted rate) for their insurance coverage.
Employees who have waived coverage as well as retirees, spouses and dependents (age 18 and older) who are enrolled in the medical portion of the SEHP may participate in programs and earn credits for the prize drawings but do not need to earn credits toward the premium incentive.

PRIZES DRAWINGS:
In addition to the premium discount, employees can win gift cards through prize drawings as follows:
- November 30, 2011Prize Drawing - Includes everyone who has registered an account since January 2010 on the wellness portal
- March 31, 2012 Prize Drawing - Includes everyone who has earned at least 20 credits
- July 31, 2012 Prize Drawing - Includes everyone who has earned at least 20 credits; those who have earned more than 20 credits will be entered multiple times for more chances to win
A total of 53 winners will be selected during each drawing! There will be 1 winner of a $100 gift card, 2 winners of $50 gift cards, and 50 winners of $25 gift cards.
