HealthQuest Rewards Program


The deadline to qualify for the 2015 premium incentive discount has been extended from July 31 to November 15, 2014.  Employees who have already qualified for 2015 can report their doctor, dental and vision appointments taking place during the extension period for credits in the next program year. They just need to wait until November 16  when the new program year begins to report these on the wellness portal.


August 1, 2013 – November 15, 2014

From August 1, 2013, through November 15, 2014, employees enrolled in the medical portion of the State Employee Health Plan (SEHP) have an opportunity to earn credits by participating in wellness activities offered through HealthQuest.  Those who complete a health assessment questionnaire worth 10 credits and earn an additional 20 credits by Nov. 15, 2014, for a total of 30 credits, will receive an incentive. The incentive is a discount of $480 on their health plan premium for Plan Year 2015.

The HealthQuest Rewards Program is optional and employees are not required to participate in order to be covered under the SEHP.  There is no additional cost added to an employee’s premium for not participating, but they will be paying the base rate (undiscounted rate) for their insurance coverage.

Employees who have waived coverage as well as retirees and spouses who are enrolled in the medical portion of the SEHP may participate in HealthQuest programs but do not need to earn credits toward the premium incentive.
