Options for Earning Credits


HealthQuest is the wellness program for State of Kansas employees and members of the State Employee Health Plan. We offer a wide variety of wellness programs to help you, and your family, improve your health and enhance your quality of life. As part of your benefits plan, these services are provided to you at no additional cost.

HealthQuest Rewards Program

From November 16, 2015, through November 15, 2016, employees enrolled in the medical portion of the State Employee Health Plan (SEHP) have an opportunity to earn credits by participating in wellness activities offered through HealthQuest. Those who complete a health assessment questionnaire (worth 5 credits) and earn an additional 25 credits, for a total of 30 credits, by November 15, 2016, will receive a premium incentive discount of $240 in Plan Year 2017.

Employees may select from a wide variety of online, telephonic and in-person programs to earn credits as shown in the chart below. Please note that the Health Assessment Questionnaire is REQUIRED to earn the premium incentive discount.

Earning Period: November 16, 2015 – November 15, 2016

Goal for Plan Year 2017 Premium Incentive Discount:

Program Details

Below are the details of how to earn credits for each HealthQuest activity. To learn more, or begin an activity, simply click the blue link that is the program title.

Biometric Screening

HealthQuest offers three ways you can obtain your biometric numbers:

1. Attend a free onsite biometric screening (worth 5 credits) - offered in many locations statewide. Events are administered with a simple finger stick test and provide you with important health information including:

  • Total cholesterol, HDL, LDL and total cholesterol to HDL ratio
  • Triglycerides and glucose
  • Systolic and diastolic blood pressure
  • Height, weight and body mass index (BMI)
  • Waist circumference

Make an appointment online  or call toll-free 1-888-275-1205 (option 5). New events are added to the schedule as needed. Your results are automatically loaded into your online health assessment questionnaire within five business days of your appointment.

2. Order and Return an at-home screening kit (worth 5 credits) - available to employees who do not have access to an onsite biometric screening event in their area. A test kit is mailed to your home upon request. You collect your own blood sample through a simple finger stick test and return your sample in a self-addressed, stamped envelope. Your results are automatically loaded into your online health assessment questionnaire within 15 business days of returning your sample. PLEASE NOTE: the at-home test kit does not include results for blood pressure, height, weight and waist circumference needed to complete your online health assessment questionnaire. 3. Use biometric results from your personal physician - employees may use results from a recent physician's visit to complete the health assessment questionnaire. The State Employee Health Plan will cover one preventive exam per calendar year in full. Print the Biometric Values Form to take with you to your appointment. Employees may use test results from visits occurring between November 16, 2015, and November 15, 2016.

Health Assessment Questionnaire - REQUIRED (5 Credits)

Using results obtained from a biometric screening, doctor visit, or at-home screening kit, complete an online Health Assessment Questionnaire to receive valuable health information and a personal health improvement plan developed just for you. The Health Assessment Questionnaire is required to earn the HealthQuest premium incentive discount. 5 credits will be awarded on the portal automatically upon completion. Up to 12 credits will be awarded on the portal automatically upon completion of the Health Assessment Questionnaire if:

  • Non-tobacco user (3 credits)
  • Blood Pressure Less Than 120/80 (3 credits)
  • Total Cholesterol Less Than 200 (3 credits)
  • Glucose Less Than 100 (3 credits)

Lifestyle Coaching (15 Credits)

Reach health goals, big and small, by working with an expert coach one-on-one by phone and email. The enhanced Lifestyle Coaching program is easy to use and more personalized to help you set goals and reach them. Lifestyle Coaching is flexible and your coach will work with you to meet your needs as goals and priorities change. It includes interactive tools, videos and integration with mobile trackers to help you stay connected. You will receive personal support from a specialized health coach, to keep you motivated and they will provide custom meal and fitness plans. The Lifestyle Coaches include experts such as social workers, behavior coaches, fitness professionals, registered dietitians and registered nurses with extensive clinical experience in behavioral health.

Enroll online or call toll-free 1-888-275-1205 (option 4). A total of six (two-way) interactions with your lifestyle coach plus completion of a survey within six months of enrollment is required to earn credits. The survey becomes available three months post enrollment. Credits will be awarded on the portal on a twice-monthly basis. If you are unable to complete the requirements before November 15, 2016, you will still earn credits when you complete the program, however, it will count for the next program cycle.

Condition Management (15 Credits)

Condition management is designed to help you manage a chronic condition. You'll receive guidance and encouragement to support your doctor's plan of care from an experienced nurse. You may also be eligible to receive specialized health monitoring devices to help you better track your progress. Condition management programs are offered to those who have been diagnosed with or receive treatment for the following:

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Coronary Artery Disease
  • Diabetes
  • Heart Failure

To earn credits for condition management you must enroll and complete at least three additional calls. Credits will be awarded on the portal on a twice-monthly basis. If you are unable to complete the requirements before November 15, 2016, you will still earn credits when you complete the program, however, it will count for the next program cycle.

Tobacco Cessation Program (15 Credits)

Enroll in Quit For Life, the nation's leading tobacco cessation program, and work with expert quit coaches by telephone to develop a personal quit plan. Nicotine therapy may be provided at no cost if appropriate. A total of five telephonic discussions with a quit coach must be completed to earn credits. Credits will be awarded on the portal on a twice-monthly basis. If you are unable to complete the requirements before November 15, 2016, you will still earn credits when you complete the program, however, it will count for the next program cycle.

Health Advisor Call (3 Credits)

The Health Advisor service is available to help you better understand your personal risk factors identified in the Health Assessment Questionnaire. Speaking with a Health Advisor you can also learn about resources to help you achieve your goals. Call toll-free 1-888-275-1205 (Option 6) and you will be connected to a Health Advisor. Credits will be awarded on the portal automatically upon completion.

Preventive Exams (5 Credits Each)

Even healthy individuals need regular exams. Make preventive screenings an annual routine to maintain your health status long term. These include the Well Woman/Well Man, dental and vision exams. Exams completed between November 16, 2015, and November 15, 2016, will be accepted. To earn credits for preventive screenings, complete the self-report forms. Credits will be awarded on the portal automatically upon completion. A maximum of 20 credits can be earned for Preventive Exams.

Wellness Challenges (5 Credits Each)

Challenges are designed to give employees the opportunity to practice healthy behaviors. Challenges focus on key behaviors such as physical activity, nutrition, weight and stress management that help to lower health risks. Research shows these behaviors enhance the health and quality of life for individuals that include them in their daily lifestyle.

These challenges are being offered in this program cycle:

Healthy Holidays (November 23, 2015 – January 1, 2016)
Water Works (January 18 – February 12, 2016)
Fast Break to Fitness (March 14 – April 8, 2016)
EAT MOVE SLEEP (May 2 – May 27, 2016)
Finding Focus (July 18 – August 12, 2016)
“The Great Outdoors” October (October 3 – October 28, 2016)

Each challenge can only be accessed through the portal during the timeframe specified. Participants track their progress daily or weekly on the portal. Credits will be awarded on the portal automatically upon completion. A maximum of 30 credits can be earned for Challenges.

Agency Wellness Programs (5 Credits Each)

Employees may earn credits by participating in wellness programs offered by their agency that have been pre-approved for credits by Health       Quest.  Agency Wellness Programs completed between November 16, 2015 and November 15, 2016, will be accepted.  To earn credits, complete the self-report forms.  Credits will be awarded on the portal automatically upon completion.  A maximum of 10 credits can be earned for Agency Wellness Programs.

Agency Training Classes (3 Credits Each)

Employees may earn credits by completing special training courses offered by their agency. Classes completed between November 16, 2015 and November 15, 2016, will be accepted. A maximum of 6 credits can be earned for Agency Training Classes.

Virtual Coaching (2 Credits Each)

This personalized, online program is designed to help you take an active role in improving your health and quality of life. Start by picking a focus area then, set a goal, build your to-do list and get started! Focus areas, goals and to-do lists are based on your health assessment information, or you can choose to tailor the program to fit your needs. Weekly motivational email reminders help you stay on track. You will have access to tips to reduce stress, tools to create customized meal plans and personalized fitness plans and a final scorecard that measures your progress.

To earn credit, you need to complete at least healthy action per week for six weeks and a check-in assessment. You may set and work on several goals at one time, but only one healthy action per week counts toward your completion of the program. Each program requires a minimum of six weeks to complete. Credits will be awarded on the portal automatically upon completion. A maximum of 10 credits can be earned for Virtual Coaching during the program year.

Online Monthly Seminars (1 Credit Each)

These fun and engaging seminars can help you learn more about important health topics and taking steps to live a healthier lifestyle. A new seminar topic is available each month. Credits will be awarded on the portal automatically upon completion. A maximum of 3 credits can be earned for online monthly seminars.

Conversations (1 Credit Each)

Are you ready to have a virtual "conversation" about your health? By answering a few simple questions, you can get personalized advice on important health topics. Try one of these fun learning tools today! Credits will be awarded on the portal automatically upon completion.  A maximum of 3 credits can be earned for conversations.

Health & Fitness Activities (1 Credit Each)

These are examples of self-reported wellness activities you can earn credits for completing:

  • Weight loss programs
  • Fitness events and activities
  • Wellness lunch and learns
  • Wellness seminars
  • Job safety training

To earn credits, complete the self-report forms. Credits will be awarded on the portal automatically upon completion. A maximum of 3 credits can be earned for Health & Fitness Activities.

Kansas Financial Learning Center Modules (1 Credit Each, Max of 5 Credits)

Credit card bills, debt, and savings may not be top of mind for you, but the financial decisions you make today will have a long-term impact on your life. The Kansas Financial Learning Center is an interactive learning program that will prepare you with the right skills to manage your financial future. For each ten-minute learning module you complete, you will earn one HealthQuest credit, up to a maximum of 5 credits. Credits will be awarded on the portal twice monthly. A maximum of 5 credits can be earned.

EAP Webinars (1 credit each)

The Employee Assistance Program is a free service which provided counseling, financial, legal and work-life support for employees and their household to receive confidential professional help in resolving personal issues. Once monthly they put on approximately 1 hour webinars that are valuable learning topics from personal issues, planning for life's events or simply managing daily life that can affect your work, health and family. Credits will be awarded on the portal once monthly. A maximum of 3 credits can be earned.

Register for Castlight Health (3 credits)

Castlight is the personalized, health care one-stop-shop. No more surprise bills when it comes to your health care. Castlight makes it easy to shop for medical services and pharmacy based on cost, quality, and convenience; and you'll know how much you should expect to pay before your visits. Credits will be awarded on the portal once monthly.

Castlight Health - Complete Quiz and Video (2 credits)

Complete a ten (10) question educational quiz about health, wellness and Castlight, then watch a video about Castlight. It can help you find a doctor, facility or prescription and see cost estimates based on your location, your health plan, and your deductible status. It shows only NETWORK options. Also, keeps track of past claims and medical spending. Credits will be awarded on the portal once monthly.

Register for RX Savings (3 credits)

Rx Savings Solutions is a consumer tool to help you save money on prescriptions medications. Rx Savings Solutions also helps you work with your physician and pharmacy to make saving money a reality. The team of pharmacists at Rx Savings have the knowledge of prescription therapies and interchanges and will alert you to savings opportunities that maintain the quality of your care while reducing your out of pocket expenses. Rx Savings Solutions will only notify you if there is an opportunity to save. Notification will be via email or text messaging, no phone calls. Credits will be awarded on the portal twice monthly.
