Options for Earning Credits


Employees may select from a wide variety of online, telephonic and in-person programs to earn credits for the HealthQuest Rewards Program. Refer to the program descriptions below the chart for details.

PROGRAM DETAILS:

Biometric Screening

HealthQuest offers three ways you can obtain your biometric numbers:

  1. Attend one free onsite biometric screening – offered in many locations statewide. Events are administered with a simple finger stick test and provide you with important health information including:
    • Total cholesterol, HDL, LDL and total cholesterol to HDL ratio
    • Triglycerides and glucose
    • Systolic and diastolic blood pressure
    • Height, weight and body mass index (BMI)
    • Waist circumference
  2. Make an appointment on the wellness portal or call toll-free 1-888-275-1205 (option 5).

  3. Use biometric results from your personal physician – employees may use results from a recent physician’s visit to complete the health assessment questionnaire.  The State Employee Health Plan will cover one preventive exam per calendar year in full (no co-pay).  Print the Biometric Values Form to take with you to your appointment.  Employees may use test results from visits occurring between August 1, 2012, and July 31, 2013.

  4. Order an at-home screening kit – available to employees who do not have access to a health screening event in their area.  Your results are automatically loaded into your online health assessment within 15 business days of returning your sample. Kits must be ordered on the wellness portal. PLEASE NOTE: the at-home test kit does not include results for blood pressure, height, weight and waist circumference needed to complete your online health assessment.

Health Assessment - REQUIRED (10 Credits)

Using results obtained from a biometric screening, physician visit, or at-home screening kit; complete an online health assessment on the wellness portal to receive valuable health information and a personal health improvement plan developed just for you. The health assessment is required to earn the premium discount.  Credit will be awarded automatically upon completion.

Non-Tobacco User Declaration (10 Credits)

Employees who do not use tobacco products can make a “Non-Tobacco User” declaration between August 1, 2012, and July 31, 2013, by completing the self-report form for non-tobacco use on the wellness portal. Credit will be awarded automatically upon completion of the form. 

Tobacco Cessation Program (10 Credits)

Enroll in Quit For Life, the nation's leading tobacco cessation program, and work with expert quit coaches by telephone to develop a personal quit plan.  Receive an 8-week supply of nicotine patches or gum at no cost to you (if appropriate).  A total of five telephonic discussions with a quit coach must be completed by July 31, 2013, to earn credit for the program. Credit will be awarded within two weeks of completing your fifth discussion with a quit coach.

Condition Management (10 Credits)

Condition management is designed to help you manage a chronic condition to achieve your best level of health. You'll receive guidance and encouragement to support your doctor's plan of care from an experienced nurse. You may also be eligible to receive a specialized health monitoring device to help you better track your progress. Condition management programs are offered to those who have been diagnosed with or receive treatment for the following:

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Coronary Artery Disease
  • Diabetes
  • Heart Failure

Call toll-free 1-888-275-1205 (option 1).  Find out if you're eligible to receive an in-home health monitoring device.  To earn credit for condition management you must enroll and complete at least three calls with a nurse.  Credit will be awarded on a monthly basis. If you are unable to complete the requirements before July 31, 2013, you will still earn credit when you complete the program, however, it will count for the next program cycle toward your 2015 premium discount.

Health Coaching (10 Credits)

Health coaching is designed to support you in making positive lifestyle changes. Call today to enroll in telephonic coaching with a personal health coach who will provide the tools, motivation and support to help you reach your goals.  Instant message chat and email communication with your coach are also available to help you:

  • Lose weight
  • Be more active
  • Ease stress
  • Eat healthier foods
  • Gain energy
  • Be more confident

Enroll on the wellness portal or call toll-free 1-888-275-1205 (option 4).

A total of eight (two-way) interactions with your health coach plus completion of a survey within six months of enrollment is required to earn credit.  Credit will be awarded on a monthly basis.  If you are unable to complete the requirements before July 31, 2013, you will still earn credit when you complete the program, however, it will count for the next program cycle toward your 2015 premium discount.

Health Advisor Call (5 Credits)

The Health Advisor service is designed to help you better understand your personal risk factors after completing the health assessment questionnaire.  By speaking with a Health Advisor you will also learn what resources are available to help you achieve your goals and get answers to your questions about the HealthQuest Rewards Program.  After completing the health assessment questionnaire, you may call toll-free 1-888-275-1205 (option 6) and you will be connected to a Health Advisor.  Credit will be awarded automatically upon completion of the Health Advisor Call. 

Healthy Living Programs (5 Credits Each)

These interactive personalized online tutorials help you take important steps to reduce risk and improve health. There are 13 Healthy Living Programs accessible on the wellness portal to choose from.  You may participate in only one of these programs at a time.  Each program requires a minimum of six weeks to complete.  Credit will be awarded automatically upon completion.  A maximum of 15 credits can be earned for Healthy Living Programs.

To earn credits toward your 2014 premium discount, you must enroll on or before June 18, 2013, and stay engaged weekly in order to complete your program by the deadline.  If you enroll after June 18, 2013, you will still earn credit when you complete a program, however, it will count for the next program cycle toward your 2015 premium discount.

Wellness Challenges (5 Credits Each)

Challenges are designed to give employees the opportunity to practice healthy behaviors. Challenges focus on key behaviors such as physical activity, nutrition, weight management and stress management that help to lower health risks. Research shows these behaviors enhance the health and quality of life for individuals that include them in their daily lifestyle.

The challenges are being offered in this program cycle:

  • Healthy Habits Challenge (August 20 – September 17, 2012)
  • Maintain Don’t Gain (November 7 – December 19, 2012)
  • New Year New You (January 16 – February 13, 2013)
  • Reach Your Peak Challenge (April 3 – May 15, 2013)

Each challenge can only be accessed through t the wellness portal during the timeframe specified.  Participants track their progress daily or weekly on the portal.  Challenges are worth 5 credits each.  Credit will be awarded automatically upon completion of each challenge. 

Preventive Exams (5 Credits Each)

Even healthy individuals need regular exams.  Make preventive screenings an annual routine to maintain your health status long term.  These include the Well Woman/Well Man, dental and vision exams. To earn credits for preventive screenings, log on to the wellness portal and click on "Self-Report Forms" from the left menu. Exams between August 1, 2012, and July 31, 2013 will be accepted. Credit will be awarded automatically upon completion of each form. A maximum of 20 credits can be earned for preventive exams.

Agency Training Classes (5 Credits Each)

Employees may earn credits toward their premium discount by completing special training courses offered by their agency: List of Approved Classes & Codes. To earn credits for training classes, log on to the wellness portal and click on "Self-Report Forms" from the left menu and then click on "Complete Forms." Classes completed between August 1, 2012, and July 31, 2013, will be accepted. Credit will be awarded automatically upon completion of each form. A maximum of 15 credits can be earned for Agency Training Classes.

Agency Wellness Program (5 Credits)
Employees may earn credits by participating in wellness programs offered locally by their agency that have been pre-approved for credits by HealthQuest.  To earn credits for agency sponsored wellness programs, log on to the wellness portal and click on "Self-Report Forms" from the left menu and then click on "Complete Forms." Credit will be awarded automatically upon completion of the form.

Governor’s Weight Loss Challenge (10 Credits)

Governor Sam Brownback is excited to announce a statewide Weight Loss Challenge for the 2013 legislative session. Recognizing that obesity has become an epidemic across the country, the Governor wants to encourage Kansans to be physically active and eat well through this fun and competitive contest.

The Weight Loss Challenge will run from January 15 through May 15, 2013. The Governor will recruit a team of five State employees who will challenge other teams of five in losing the greatest percentage of total starting weight.  Registration begins December 17, 2012, and must be completed by Monday, January 14, 2013 at www.weightloss.ks.gov.

Online Monthly Seminars (2 Credits Each)

These fun and engaging seminars can help you learn more about important health topics and taking steps to live a healthier lifestyle. A new seminar topic is available each month on the wellness portal. Credit will be awarded automatically upon completion of each seminar.  A maximum of 6 credits can be earned for online monthly seminars. 

Conversations (2 Credits Each)

Are you ready to have a virtual "conversation" about your health? By answering a few simple questions on to the wellness portal, you can get personalized advice on important health topics. Try one of these fun learning tools today!  Credit will be awarded automatically upon completion of each seminar.  A max of 6 credits can be earned for conversations. 

Health & Fitness Activities (2 Credits Each)

These are self-reported wellness activities you can earn credits for.  Examples include:

To earn credits, log on to the wellness portal and click on "Self-Report Forms" from the left menu. Credit will be awarded automatically upon completion of each form.  A maximum of 6 credits can be earned for Health & Fitness Activities.

Nurse 24 (2 Credits)

The nurse line is available 24 hours a day, 7 days a week for any health related question. Call toll-free and speak confidentially with a specially trained nurse any time, day or night. From general health and medical information to urgent issues, Nurse 24 can help you make the most informed decisions about what to do. Just call toll-free 1-888-275-1205 (option 2).

Credit can be earned for using the Nurse24 service. You must self-report your participation on to the wellness portal by completing the Health & Fitness Activity self-report form (see above). Credit will be awarded automatically upon completion of the form. A maximum of 2 credits can be earned for using Nurse24. 

Wellness Portal

The wellness portal is an online tool that features a credits tracking system that makes it easy for you to keep track your credits earned and to see how close you are to your goal of 30 credits in order to receive the 2014 premium discount. Log on to the wellness portal for the most up-to-date health and wellness information 24 hours a day. Start clicking around to discover many valuable resources at your fingertips!
