Take the online health assessment to better understand your current health status and receive a personalized report and health improvement plan developed just for you. In order to generate your plan, the assessment analyzes your answers to a series of questions that take about 20 minutes to complete.
All eligible participants who complete the online health assessment between October 1, 2011 and July 31 , 2012 using results obtained from a health screening will earn 10 credits for the HealthQuest Rewards Program!
Before beginning your online assessment, you will need your biomtric values from a health screening. HealthQuest offers three options to obtain your biometric values - click on the health screening link above for details on these three options. These screening values you will need are:
- Total cholesterol, HDL, LDL
- Triglycerides and glucose
- Systolic and diastolic blood pressure
- Height and weight
- Waist circumference
When you are ready to begin your online assessment, go to the wellness portal at www.KansasHealthQuest.com and log into your account or register a new account if you do not already have one. Then click on “Health Assessment” from the menu on the left and select “Online Health Assessment.”

Who is eligible to participate in the online health assessment?
- Benefits eligible State and Non State employees who are enrolled in the State Employee Health Plan or who have waived coverage in the plan
- Retirees, spouses and dependents 18 years or older who are enrolled in the medical portion State Employee Health Plan
