Earning Period: January 1, 2017 – December 31, 2017

Complete the online health assessment questionnaire to better understand your current health status and receive a personalized report and health improvement plan developed just for you. In order to generate your plan, the assessment analyzes your answers to a series of questions that take about 10-15 minutes to complete.

All eligible participants who complete the online health assessment between January 1, 2017, and December 31, 2017, using results obtained from a biometric screening will earn 10 credits for the HealthQuest Rewards Program!

Before beginning your online assessment, you will need your biomtric values. HealthQuest offers three options to obtain your biometric values.

The values you will need are:

  • Total cholesterol, HDL, LDL
  • Triglycerides and glucose
  • Systolic and diastolic blood pressure
  • Height and weight
  • Waist circumference

When you are ready to begin your online assessment, log into your wellness portal at https://kansashealthquest.cernerwellness.com and Click “Step 1“ in the Image Slider at the top of the page.

Because the health assessment questionnaire is required for the premium discount, a paper version is available.  Employees can request a paper health assessment questionnaire by calling the Help Desk toll free at 1-888-275-1205, option 3. Completed forms must be returned with a postmark date no later than November 1, 2017. We strongly encourage employees to refer to a friend or family member with a computer or the local library for free access to the Internet.  Going online is the fastest and easiest way to complete health assessment questionnaire and receive credit immediately.  The paper form should be utilized as a last resort.
