Immunization Action Plan (IAP)


Dear Local Health Department Immunization Partners,

All counties who have a Vaccine For Children Program in the local health department are eligible to apply. If the county health department is a recipient of the MCH program, the IAP application is part of the MCH application and the funding will be distributed through the MCH award as has been discussed in both MCH application instructions and through What's Happening Wednesday in recent weeks. If the local health department is not a MCH direct recipient, the health department will need to complete the IAP application independently in Catalyst.

Fund allocations have been determined by formula for this upcoming year and will continue to be in years going forward. Here is a description of how the formula has been applied.

1. Base Funding: Each county has base funding of $1,250.00. This is to provide support for at least one staff person from each county to attend the Kansas Immunization Conference in 2018 and to support core clinic functions.

2. Per Medicaid Population Allocation: Each county is provided $0.87 for each child enrolled in their county for Medicaid or CHIP in 2016

3. Per Dose Allocation: Each county is allocated $1.57 for each dose of VFC or CHIP administered in 2016 by the local health department.

Excess funds left from counties who choose not to request IAP funding will be redistributed to other counties.

Quarterly

Submit the “Certified Affidavit of Expenditures” in Catalyst. Refer to the Fiscal Reporting Requirements document.

BI-Annually (October 15 and April 15)

Complete the Semi-Annual Progress reports in Catalyst. D-1 will be completed in October and D-2 will be completed in April.
