Click the “Submit Form” button or email the “Certified Affidavit of Expenditures” to Kevin Shaughnessy at KShaughnessy@kdheks.gov. State grant funds cannot be used to purchase Capitol Equipment.
Quarterly Program Report
Submitted Pregnancy Maintenance Initiative Report to Jane Stueve at jstueve@kdheks.gov
Bi-annually Program Report (January 15 and July 15)
Submit a narrative of actions/revisions/etc to ensure achievement of the objective by the end of the contractual period. Submit to Jane Stueve at jstueve@kdheks.gov
