Program Purpose

To provide comprehensive services to low-income and high-risk families in Geary and Wyandotte counties, utilizing a public health targeted case management model


Eligible Applicants

Not-for-profit organizations in Geary and Wyandotte counties


Funding Information

Grants will be awarded annually on a competitive basis. Award amounts are contingent upon State General funding available and allocated for this initiative. No part of the grant money shall be used for any political purposes. Priority is given to continue funding of local agencies that consistently meet contract objectives, reporting requirements.


Grantees will use these state funds as matching dollars to draw down Medicaid dollars to provide services to KanCare enrolled women and children in their county.
