Mid and Final – Year Reporting Requirements

All applicants must complete mid-year and final reports. Progress reporting is to be completed within the Catalyst system (www.Catalystserver.com) under the requirement and work plan sections for each approved Chronic Disease Risk Reduction activity.

The mid-year report and affidavit of expenditures for the period of July 1 through December 2015 will be due no later than January 15, 2016.

The final report and affidavit of expenditures for the period of January 1 through June 30, 2016 will be due no later than July 15, 2016.

Grantees will be required to participate in two formal site visits taking place in the first and third quarters of the fiscal year.

Additional instructions on how to report progress on Chronic Disease Risk Reduction activities with in Catalyst will be posted by October 15, 2015.
