Hospital Preparedness Program Grant Materials


National Incident Management System (NIMS) Resources for Hospitals

Certain local health department and hospital personnel are required to be trained in NIMS. These forms should be completed and kept up to date, and can be used for documentation purposes. Community Hospitals are required to complete and submit the NIMS Compliance Tool to the Planning & Technology Specialist.


Alternate Care Sites

An Alternate Care Site (ACS) is a community-based off-site location that can provide a minimum specific level of care in response to a mass casualty incident creating a large number of people seeking medical assistance. The community ACS template and tools below can be used by a community to increase local response capabilities through interagency coordination and planning.


Hospital Hazard Vulnerability Assessment (HVA)

Hospitals should collaborate with local law enforcement, emergency management, and other community partner agencies to complete the HVA and Gap Analysis forms. Hospital preparedness grant funds can be used to minimize identified “gaps” in planning, equipment, training, exercise, or other areas. Community hospitals are required to complete an HVA and GAP Analysis every other year. An updated HVA and Gap Analysis are required for the 2011-2012 grant. The completed HVA and Gap Analysis must be submitted to the Preparedness assigned planner for approval (see Planning & Development Review Team Map for assigned reviewer).
