rural communities through the development of locally controlled community-based
educational interventions to accomplish certain behavioral and educational
objectives.” Projects strive to help teens achieve their full potential
through a positive youth development approach.

guidance is attached. Applications will be accepted on or before April 5, 2007.
Teen Pregnancy Case Management Projects offer
case management services to pregnant or parenting teens receiving
Medicaid in an effort to improve self-sufficiency and goal formation
of teens. Delaying the birth of a second child is attempted through
goal counseling, peer support groups and assistance in locating
and using educational, health, social service and other community
resources. Case managers provide intensive support to teenagers
in completing their education, seeking employment, accessing health
services, and guiding them through the maze of decisions and obstacles
that they face. Services continue until the adolescent leaves the
program at age 21 or upon achieving goals set out in the case management
plan. Projects are located in Douglas, Geary, Montgomery, Sedgwick and Wyandotte
Counties.
