1. Print survey form. (WORD)  -  (PDF)    
   Gather the data requested in the survey. Enter the data into the electronic survey instrument for submission.
2. This survey should take less than 20 minutes to complete. Please note that you must complete the survey in a single session; you will not be able to save some answers and finish at a later time. We are asking for your completed response by June 1, 2008.
3. All school nurses that have any contact with the students need to complete the nurse survey.
