Collaborative Model


The perinatal community collaborative model is driven by private and public partnerships at both state and local levels, including: Title V Maternal and Child Health (MCH) (public health), Medicaid, private foundations, local health departments, federally qualified health centers, clinical providers, local hospitals, and community and faith-based organizations. The community collaborative model brings permanent MCH infrastructure, leveraged and shared resources, change in the prenatal care delivery system, a vehicle to identify community needs, a standardized evaluation system, and new opportunities for achieving community collective impact and improved birth outcomes. Coordination and collaboration is essential, as participants are scheduled into the program during prenatal visits. Partnerships at the local level leverage funds and other resources to support sustainability.

Goal of Community Collaboration
The goal of collaborations is to bring individuals, agencies, organizations and community members themselves together in an atmosphere of support to systematically solve existing and emerging problems that could not easily be solved by one group alone. Collaborations should focus on increasing capacity, communication and efficiency while improving outcomes.

What is Collaboration?
Collaboration is a process of participation through which people, groups, and organizations work together to achieve desired results. Collaborations accomplish shared vision, achieve positive outcomes for the audiences they serve, and build an interdependent system to address issues and opportunities. Collaborations also involved the sharing of resources and responsibilities to jointly plan, implement and evaluate programs to achieve common goals. Members of the collaboration must be willing to share vision, mission, power, resources and goals. Community collaboratives comprised of partners sharing resources and rewards serve as the backbone for program implementation, thus leading to a greater collective impact.
